One service at a time here. A real IEP usually promises several — speech, occupational therapy, resource minutes, and often a separate accommodation list — each with its own count, plus goal-progress dates and meeting deadlines. The full spreadsheet tracks all of them together.
Because a conversation about 'it feels like sessions keep getting missed' goes nowhere, and a page showing 540 delivered out of 720 owed does not. A dated record is the difference between a worry and a fact.
Anything the IEP promised that did not happen: a cancelled session, a therapist absence with no make-up, a session cut short. Note the reason next to it - patterns matter more than single days.
